A Transition Planning Committee (TPC) is a consortium of local members of schools, agencies, and not-for-profit organizations, parents of children with disabilities and people with disabilities who collaborate to improve the transition life-cycle from high school to a successful adult life. The Transition Planning Committees are powerful tools for improving the local capacity for delivering effective services for students with disabilities in local communities.
